The recent cleanup in HtmlWriter has meant that there is little significant functionality left in HtmlWriter itself, and what remains would be better as part of the HtmlDocument class.

This will simplify the code in a number of places, and will eliminate a now obsolete superclass of HtmlDocletWriter.
