The spec for ByteLookupTable should document the exceptions
thrown by methods lookupPixel(*) when the input parameter src values are
out of range. The spec should also define the valid range
of inputs. The spec should also document exceptions thrown and conditions
when there is a mis-match between the size of src and dst input parameters.

This spec bug also applies to ShortLookupTable
