When stepping continuously (that is, the same step request generates multiple
step events - no addCountFilter), starting from a point at which there are
no frames on the stack, frame exits are not handled are thus step
sequencing is incorrect or in some cases step ceases to occur (see attached
test case).

Occurs in Kestrel and early Merlin.
