Some Portable Interceptor code needs to be revisited before Merlin ships. The following items, in particular:

1. Make ClientRequestInfoImpl constructor protected
2. InterceptorList: remove unused interceptorClassPackage attribute
3. Change RequestInfoImpl.insertApplicationException to handle reset() failure
   case with an INTERNAL.
4. ORBInitInfoImpl.resolve_initial_references: Add minor code and reason
   String to INTERNAL.
5. Remove public access modifiers from PICurrent methods.
