Add an option such as

    -noqualifier java.lang:java.applet

that indicates which package names would not have fully qualified classes
in these places:

    parameter types
    return types
    @see and @link tags

The syntax could be similar to that used for -group. Allow wildcards
for convenience so the list does not have to be very long.
