XSLTC's output system has a number of problems.

1. It does not allow character escaping to be passed through a chain of multiple transformations.

2. cdata handling in conf test copy32 is not correct.

3. Processing instructions are not supported with SAX output.

4. Character encodings, such as Shift_JIS, are not being handled
   (bugzilla 7679).

5. Comments with terminating ---> not escaped (bugzilla 6925).
