Original spec has different field index kinds used for static vs instance fields. Three major VM vendors have implemented using only one kind. The two known tools have implemented using only one kind. Problem must be resolved.
JVMTI Spec: fix JVM TI 1.0 and 1.1 spec such that only a single kind of field index is used
