HPROF: Create hprof dumps direct from FollowReference callbacks

Remove use of hprof_reference.c saving of data during FollowReference callbacks, make calls to hprof_io,c functions direct from the callbacks.
The heap snapshot work described in https://heap-snapshot.dev.java.net/ should address this, hopefully we can create this new format this way.
