Here are some minor improvements to Exchanger:

- better method names in sample code
- provide URL for Exchanger paper
- generify "arena"
- use more standard coding style
- waitForHole can save a memory barrier if CAS succeeds; so test it
- Some methods have ambiguous "it" not referring to obvious antecedant.
