No concrete steps to reproduce - very random. It happened to me that when running NetBeans with embedded WebView, the main NetBeans menu disappeared on Mac. After this happened, and I switched to another application (Chrome) and back to NetBeans, the main Mac menu was still showing Chrome and didn't change back to NetBeans menu. I am attaching a message from the NetBeans log file which I suspect is related.
