Eliminate HtmlDocWriter

      With recent cleanup, there is almost nothing left in HtmlDocWriter, and we can finally reduce the confusing triumvirate of HtmlDocletWriter <: HtmlDocWriter <: HtmlWriter down to just HtmlDocletWriter.
