Remove IgnoreLibthreadGPFault

      IgnoreLibthreadGPFault was added as a develop flag to be able to suppress an Solaris IA32-only error back in early 2003. As Solaris IA32 is no longer a supported platform the flag can be removed along with the workaround code it suppress.
