SA ClassWriter generates invalid invokedynamic code

      Invokedynamic instructions generated by the SA ClassWriter do not refer to valid constant pool entries.

      More strict constant pool control in javap reveals that `serviceability/sa/ClhsdbDumpclass.java` test fails with errors (see attached log).
